Services > Disability Determination Services > Filing a Social Security Disability Claim

What to Bring

Social Security and DDS can often process claims more quickly if you take certain records and other information to the application. However, do not wait to file for disability, even if you do not have all of this information.

The date work stopped and a description of how the impairment(s) prevents work.
Names, addresses, phone and fax numbers of treating doctors, hospitals and clinics. Include dates of treatment.
Copies of medical records, if available. These could include office notes and summaries, lab tests, x-rays and other relevant studies.
Names and dosages of prescribed medications.
A thorough work history, including dates worked, duties performed and names of employers for the last 15 years.
If the application is for a child, copies of report cards, IEPs, ARD conferences and psychological reports
Name and address of a friend or family member who knows about your medical condition and is willing to assist, if needed.
