How to File

DDS Mail Room

You may apply for SSDI or SSI benefits by calling SSA’s toll-free number (1-800-772-1213) or visiting a local Social Security office. To find the office nearest to you online, go to the Social Security Office Locator.

Social Security Administration representatives will assist in completing the application, and will review it to see if you are eligible to apply for disability benefits.

If so, the Social Security office will forward your case record to the DDS.
